Reporting Safety or Quality Issues
VA Central Western Massachusetts Healthcare System encourages the active involvement of patients and their families in the patient’s care as a patient safety strategy. Patients should talk to their provider about safety concerns. If they do not feel comfortable doing that, they may contact the Patient Safety Manager, Elaine Kersten, EdD, at (413) 584-4040, Ext. 2007.
