About the Program

The Primary Care Service Line for VA Central Western Massachusetts offers an extensive range of treatments with emphasis on healthy living and wellness. 

As the VA shifts from a focus on disease management to a patient-centered focus on the health of Veterans, we have developed new clinical programs to support health promotion and disease prevention, or "HPDP." This preventive care program ensures a more holistic approach and greatly improves your access to, and the coordination of, your care.  The program provides you, the patient, with comprehensive health education, clinical services and support for self-management of health-related behaviors.

You may hear this referred to as the Patient Aligned Care Team, or "PACT" initiative, which is a partnership of providers, nurses, and other healthcare professionals, working with you to help you make informed choices about your healthcare.

Services Offered

Primary Care services are available at the VA Central Western Massachusetts main campus in Northampton, and at our community based outpatient clinics in Fitchburg, Greenfield, Pittsfield, Springfield and Worcester. 

Ancillary Services include dental, radiology, ultrasound, CT, rehabilitative medicine, laboratory, respiratory therapy, pulmonary lab, pharmacy, podiatry, nutrition, speech pathology, orthopedic and ophthalmology clinics.
